*0-18 years old, must have at least one adult membership to qualify. Routine vaccines not included.

** Discounts available for families (with same address), college students (current school ID required), and small businesses.

*** Labs, IV hydration, and Pathology Services Fees are not included in visits for minor bumps or lacerations.

We do not bill insurance, Medicare, or Medicaid. It does not cover catastrophic events like hospitalization, surgery, etc. Patients are recommended to obtain at least a high deductible insurance plan or healthcare sharing plan for emergencies. DPC is a unique concept when compared to the standard traditional primary care model.
